Questionnaire on Derivative Products Knowledge

1) A certified true copy of business registration certificate by Solicitor or Practising CPA

2) A certified true copy of the certificate of incorporation by Solicitor or Practising CPA

3) A certified true copy of Articles of Association by Solicitor or Practising CPA

4) Certified true copies of register of directors by Solicitor or Practising CPA

5) A certified true copy of register of members   by Solicitor or Practising CPA

6) A certified true copy of group chart with shows your relationship with you group company(ies) shareholder and ultimate beneficial owners by Solicitor or Practising CPA, if applicable

7）A certified true of incumbency by Solicitor or Practising CPA (for overseas company)

8) Board resolutions

9) I.D. card or passport copies of all directors and authorized persons

10) Residential address proof of all directors and authorized persons issued within the last 3 months such as utility bill or bank statement

11) Lastest annual return filed with the Companies Registry (for HK registered company)

If send us your account opening form by POST:

According to the Guideline provided by SFC, if you cannot return the Account Form in person, please arrange for authentication by an appropriate person to countersign on the Account Opening Form(s), your copy of I.D. Card or passport together with his/her full name and title and company name for identity verification purpose.
Authentication should be performed by a registered person, a Justice of Peace, or a professional person such as branch manager of a bank, certified public accountant, lawyer or notary public.
